These Official Rules govern the [Sweepstakes Name] (the “Sweepstakes”). By participating, entrants agree to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding in all matters relating to the Sweepstakes.
1. Sponsor
The Sweepstakes is sponsored by the Bridge Street Merchants d/b/a AMI South End (“Sponsor”), located at [Mailing Address], Bradenton Beach, FL 34217. Co-sponsors and participating merchants, if any, will be identified in the Sweepstakes promotional materials.
2. Eligibility
The Sweepstakes is open to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older at the time of entry. Employees, officers, and directors of the Sponsor, the Bridge Street Merchants, participating merchants, and their respective parents, subsidiaries, affiliates, agencies, and immediate family members (spouse, parent, child, sibling, and their respective spouses, regardless of where they live) and persons living in the same household, whether or not related, are not eligible to enter or win. Void where prohibited by law.
3. Sweepstakes Period
The Sweepstakes begins on [Start Date] at [Start Time] Eastern Time (“ET”) and ends on [End Date] at [End Time] ET (the “Sweepstakes Period”). The Sponsor's computer is the official timekeeping device for the Sweepstakes. Entries received before or after the Sweepstakes Period will not be eligible.
4. How to Enter

Limit one (1) entry per person and per email address for the duration of the Sweepstakes Period. Multiple entries from the same person or email address beyond the stated limit will be void. Sponsor is not responsible for lost, late, incomplete, illegible, undelivered, misdirected, garbled, or postage-due entries; or for any failures of the Sweepstakes website during the Sweepstakes Period; or for any technical malfunction or other error of any kind. All entries become the property of the Sponsor and will not be returned or acknowledged.
5. Prize
One (1) Grand Prize winner will receive: [Prize Description]. Approximate Retail Value (“ARV”): $[Amount]. The prize is awarded “as is,” with no warranty or guarantee, either express or implied. No substitution, transfer, assignment, or cash equivalent of the prize is permitted, except at Sponsor's sole discretion. All federal, state, and local taxes on the prize are the sole responsibility of the winner. The winner may be required to complete and return an IRS Form W-9 prior to receiving the prize.
6. Winner Selection and Notification
One (1) winner will be selected in a random drawing on or about [Drawing Date] from among all eligible entries received during the Sweepstakes Period. The drawing will be conducted by the Sponsor or its designated representative, whose decisions are final and binding.
The potential winner will be notified by email at the address provided on the entry form within [Notification Period] business days of the drawing. If the potential winner does not respond within [Response Period] days of the notification attempt, or if the prize notification or prize is returned as undeliverable, the potential winner will forfeit the prize and an alternate winner may be selected from the remaining eligible entries.

8. General Conditions
By entering, each entrant agrees to release, discharge, and hold harmless the Sponsor, the Bridge Street Merchants, participating merchants, and each of their respective parents, subsidiaries, affiliates, officers, directors, employees, and agents from any and all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons or property which may be sustained in connection with participation in the Sweepstakes or the receipt, use, or misuse of any prize.
Sponsor reserves the right, in its sole discretion, to modify, suspend, or cancel the Sweepstakes at any time and for any reason, including without limitation if any factor interferes with the proper conduct of the Sweepstakes as contemplated by these Official Rules, or if the Sweepstakes is compromised by virus, bug, fraud, technical failure, or other causes beyond the Sponsor's reasonable control.
9. Use of Personal Information
Information submitted by entrants will be used for the purpose of administering the Sweepstakes. By entering, you also consent to receive email communications from Sponsor and participating Bridge Street Merchants members about events, promotions, and news from the AMI South End. You may unsubscribe at any time. Personal information will otherwise be handled in accordance with the Sponsor's [link to Privacy Policy].
10. Publicity
Except where prohibited by law, by accepting the prize, the winner grants the Sponsor and its designees the right to use the winner's name, likeness, photograph, voice, opinions, and biographical information for advertising and promotional purposes in any media now known or hereafter developed, worldwide, in perpetuity, without further compensation, notification, or consent.
11. Disputes and Governing Law
Any and all disputes, claims, and causes of action arising out of or in connection with the Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and shall be governed by, and construed in accordance with, the laws of the State of Florida, without regard to its conflict-of-laws principles. The exclusive jurisdiction and venue for any such disputes shall be the state and federal courts located in Manatee County, Florida.
12. Winner List
For the name of the winner, send a self-addressed, stamped envelope, after [Drawing Date], to: AMI South End Sweepstakes Winner List, [Mailing Address], Bradenton Beach, FL 34217. Requests must be received no later than sixty (60) days after the end of the Sweepstakes Period.
